Medications at school
Students that will need to take prescription medication while at school are required to have request to administer medication form signed by physician. (see form link below)
** ALL MEDICATIONS must be brought to the nurse in person. Please do not send any medications with your student or in their backpack. Please note the medication procedures on this form regarding requirements for a medication at school.
Medication prescribed or requested to be given three times a day or less will not be given at school unless a specific time of administration during school hours is prescribed by a physician. * Typically three times a day meds are given at home before school, when the student gets home, and before bed.
Anaphylaxis
Students with severe life threatening allergies (anaphylaxis) requiring EpiPen at school will need the following forms signed by physician:
FARE - Anaphylaxis action plan * If the FARE form is signed by parent and doctor, then seperate medication authorization forms (#2) are NOT required
Medication authorization form : Two are needed (1 for EpiPen & 1 for antihistamine)
Asthma
Students who require an inhaler at school for asthma will need the following forms completed and signed by a physician:
Immunization requirements
A handwritten immunization record should be considered valid if it contains the vaccine name, vaccination date (month, day, and year), and medical or public health personnel validation. Validation includes a signature, initials, or stamp.
An immunization record generated from an electronic health record must include clinic contact information and the provider’s signature/stamp, along with the vaccine name and vaccination date (month, day, and year).
Official records provided by a student’s previous school (Texas or other state) are also considered valid documentation for immunizations, as long as the date includes the month, day, and year.

WHEN TO KEEP YOUR CHILD HOME FROM SCHOOL
The American Academy of Pediatrics, the Texas Department of State Health Services, and RRISD recommend that students be kept home from school if any of the following conditions exist:
Fever: Students with a temperature of 100˚F or above will be sent home. To prevent the spread of a contagious disease, students with fever must be symptom-free and fever-free without the use of temperature reducing medications for 24 hours before returning to school.
Signs of severe illness, including fever (100 degrees F or higher), irritability, difficulty breathing, crying that doesn’t stop with the usual comforting, or extreme sleepiness.
Diarrhea: 3 or more episodes in 24 hours, and/or unable to tolerate fluids/ food. Students must be diarrhea free for 24 hours without use of medications.
Vomiting two or more times in 24 hours, unless a physician feels the cause of vomiting is not an infectious disease, and writes a note to that effect. Student must be able to tolerate food and fluids.
Fever, rash, or a change in behavior, until a physician has determined that the problem is not caused by an infectious disease.
Pink eye / Conjunctivitis : A note from the doctor / health authority who diagnosed should be turned into the school with a return date OR until symptom free.
Head lice (pediculosis): If live lice are found on a student’s head by the nurse, a letter will be sent home to the student’s parent and to the entire classroom alerting other parents to check their children for lice. If nits are found, a letter will go home only to the parent of the student.
Head lice do not jump or fly and are most commonly contracted by head-to-head contact. Head lice are commonly found in school-aged children, so parents/guardians should routinely monitor their students for live lice at home. Round Rock ISD Lice Protocol. More information on head lice can be obtained from the DSHS website Managing Head Lice in School Settings and at Home.Crutches / wheelchairs: Round Rock ISD does not provide crutches or wheelchairs for use at school. Students are required to submit a note to the school/campus nurse from a health care provider to be allowed to bring and temporarily use crutches/wheelchair in the school setting. The health care provided note should include the anticipated length of time the student will be needing the use of crutches/wheelchair along with the length of time the student needs to be excused from PE and/or recess.
